TERMS AND CONDITIONS
1. Generala. By signing the Universal Childcare Enrolment Form I hereby state that I am the person with the care, custody and control of the child whose name appears on the Enrolment Form.
b. I acknowledge that Universal Childcare is in the business of providing childcare facilities and services, both at its premises, and at premises which it might own, lease or possess.
c. I agree to accept any carer provided to me by Universal Childcare and to accept the childcare facilities provided by Universal Childcare at its premises.
d. I hereby delegate the custody and control of my child to Universal Childcare or its employees whilst Universal Childcare is caring for my child.
e. I acknowledge that Universal Childcare warrants that it will at all times and to the best of its ability use all reasonable care, concern and control in respect of caring for my child, but I acknowledge the inherent and unforeseeable dangers and difficulties in providing childcare facilities for children.
f. I authorise Universal Childcare to take all and any such action as it may consider necessary, appropriate and in the best interests of my child in all circumstances to protect my child and/or any other children at Universal Childcare's premises or under its control.
2. Medical
a. I understand and accept that should the Universal Childcare staff consider my child contagious or too ill to attend the centre that this decision be regarded as final and my child will be collected promptly from the centre. In the case that a child is unwell, the team will ensure that the child is kept as comfortable as possible until they can be picked up by a parent / guardian. This may involve the child being given a tepid bath if required.
b. I understand and accept that should my child have a contagious illness, I will not return my child to the centre until the duration of the centre clearance period or until a medical certificate is issued by a qualified and registered medical practitioner.
c. In the event that my child's temperature is at or above 38oC and Universal Childcare is unable to contact the parents, guardians or emergency contacts listed on the Enrolment Form, I hereby grant permission to Universal Childcare, its employees or agents to give one (1) only dose of paracetamol at the appropriate dosage indicated on the medication.
d. In the event of any illness or injury to my child whilst my child is in the custody and control of Universal Childcare or its employees, I hereby authorise Universal Childcare and/or its employees to seek urgent:
i. medical or dental treatment from either the doctor or dentist nominated on this Enrolment Form (or another doctor or dentist) or hospital treatment or ambulance service, or
ii. assistance from some other person or body nominated by any parent or emergency contact listed on this Enrolment Form.
iii. If, in the opinion of Universal Childcare and/or its employees, servants or agents, it is necessary to do so PROVIDED THAT Universal Childcare shall inform me as soon as possible in all circumstances of the illness or injury to my child.
e. I consent that in the event of an outbreak of a vaccine-preventable disease at the centre, the management has to notify the Department of Health of any un-immunised children in the centre and that if my child is not immunised she/he may be excluded from attendance for such time as the Department deems necessary and that the daily fee must still be paid. f. I consent to first aid being administered by a Universal Childcare employee who is the holder of a current first aid certificate or is a registered nurse.
g. I understand and accept that the Centre Director or other designated Universal Childcare staff member can only administer medication to my child if the medication has been authorised by one or other of the parents and a qualified and registered medical practitioner and if the details of the medication and its administration have been accurately recorded in the Medication Authorisation Form.
h. I hereby agree to reimburse Universal Childcare and/or its employees against any charges, costs or expenses incurred by them in obtaining such medical or hospital treatment as is referred to above.
i. Following the recommendations of the Cancer Council, I request that staff at the centre apply an SPF30+ sunscreen to all unprotected areas of skin on my child as they deem necessary.
j. I request that staff apply non-prescription insect repellent, nappy rash cream, moisturiser and/or teething gel as they deem necessary.
k. I authorise the team at the centre to check my child's head if they are concerned about the presence of head lice. This will be conducted in a respectful manner.
3. Authorisations
a. I consent for the Universal Childcare staff to take my child on local excursions which do not include any type of transportation or the crossing of main roads.
b. I consent to my child being photographed and his/her first name being used for the following purposes including (however not limited to) centre programming (for display in the centre) or group or individual records, photo displays & other documentation around the centre, artwork (group or individual), dietary &/or allergy charts.
c. I consent for the Centre Director to liaise with the Intervention Service (if applicable) in order to provide the most beneficial program for my child. I understand that she/he will notify me of such contacts as they occur.
d. I consent that my child may be collected from or brought to the centre by those listed in the 'Authority to Collect' section and/or those identified in 'Emergency Contacts' section. I understand that any person collecting my child will need to be introduced to the appropriate team members and/or provide photo identification prior to my child being released to them. I shall advise the centre whenever my child is to be collected by one of these people.
4. Fees and other charges
a. In consideration of the care provided by Universal Childcare and/or its employees, servants and agents I shall pay Universal Childcare the childcare rates which are applicable from time to time and in the event of my failure to make such payments I acknowledge that Universal Childcare shall be at liberty to terminate forthwith the provision of childcare facilities and/or services for my child.
b. I acknowledge that an Initial Payment of 4 weeks gross fees are required when placing my child into a centre and that these fees must be paid prior to my child attending at the centre.
These fees are:
i. held by Universal Childcare for the duration of your child's enrolment.
ii. non refundable when enrolling my child into an existing Centre.
iii. refundable until the date a new Centre receives its License to operate from the Department of Community Services.
iv. initial payment will be refunded within 14 days of the enrolment ending when all outstanding monies have been paid.
c. I understand that my child has a confirmed enrolment in the room and on the days as offered in the enrolment confirmation letter. The opportunity to transition to the next age group will be offered based on availability; in some cases children are unable to transition immediately after their birthday. If this is the case then my child will continue in their current room, and the usual daily fee will apply, until the transition is possible.
d. I acknowledge that a daily fee is payable for each day in which my child is enrolled and is payable for the reservation of the place not the attendance of my child, in accordance with Universal Childcare fee policy. I understand that fees are payable for public holidays or designated centre closure days which fall on days my child is enrolled and when my child is absent for any reason. I understand fees charged may change during the time in which my child is enrolled in care.
e. I acknowledge that a minimum of 48 hours notice is required for cancelling a Casual Day. Should I provide less than this required period, I understand I am liable for the full fees for the booked Casual Day.
f. I acknowledge that childcare fees are to be paid by Direct Bank Debit or by Credit Card and I will provide and maintain the necessary details and authorisation to allow Universal Childcare to organise this. I understand a $50 dishonour fee will be charged if insufficient funds are in my bank account when fees are processed or when my credit card is rejected by the bank or if we are unable to collect fees from your nominated account at the nominated time.
g. I acknowledge that any waitlist fee paid is a non refundable administration charge and is not offset against any fees due.
h. I acknowledge that any enrolment fee paid is a non refundable administration charge and is not offset against any fees due.
i. I acknowledge and agree that I shall reimburse Universal Childcare and/or its employees, servants and agents for all and any out of pocket expenses reasonably incurred in the course of the care of my child.
j. I agree that I shall give Universal Childcare notice of my intention to withdraw my child for Universal Childcare's care, not less than the equivalent of 8 weeks' worth of fees (not including the end of year closure period). I agree that should I give Universal Childcare less than the required written notice period I shall still be obliged to pay Universal Childcare an amount equivalent to 8 weeks' worth of gross childcare fees (excluding Child Care Benefit, Jet fee assistance or third party payers) otherwise payable by me to Universal Childcare based on my child's usage of Universal Childcare's childcare centre.
k. I agree that I shall give Universal Childcare notice of my intention to alter my child's days of attendance, not less than the equivalent of 4 weeks' worth of fees (not including the end of year closure period). I agree that should I give Universal Childcare less than the required written notice period I shall still be obliged to pay Universal Childcare an amount equivalent to 4 weeks' worth of gross childcare fees otherwise payable by me to Universal Childcare based on my child's usage of Universal Childcare's childcare centre.
5.Other
a. I agree that I shall neither procure nor obtain nor attempt to procure or obtain the services of any employee, servant or agent of Universal Childcare as an individual and not as an employee of Universal Childcare without first obtaining the consent of Universal Childcare. This provision shall continue for a period of 12 months from the date of my last utilising the services of any particular employee, servant or agent of Universal Childcare. Should I retain any employee or former employee of Universal Childcare in breach of this provision I acknowledge that Universal Childcare shall be entitled to claim from me the full rates which I would otherwise have had to pay to Universal Childcare for my utilising the services of the employee, servant or agent.
b. In this agreement the reference to an employee of Universal Childcare shall include any servant or agent or employee whether casual, part time or full time who indicates or represents himself/herself to be an employee, servant or agent of Universal Childcare.
c. I understand that Universal Childcare will maintain the confidentiality of my child's address, telephone number, medical details and developmental records. I also understand that any information provided via the various communication charts such as sign in/out, medication, day sheets, etc is usually on display at the centre and therefore does not remain confidential.
d. I acknowledge that I have a responsibility to treat all Universal Childcare team members with respect and behave appropriately at all times when dealing with any servant or agent or employee. Swearing, raised voices and rudeness directed at team members will not be tolerated and I understand my child's enrolment may be terminated if these terms are not adhered to.
e. I understand that Universal Childcare can choose not to accept my child into care if these agreed terms are not met or if Universal Childcare believes they are unable to adequately care for my child due to any special needs, medical or behavioural problems. I understand that Universal Childcare reserves the right to terminate, without notice, my child's position at the centre if it believes it is in the best interests of the child, other children in the centre or staff at the centre resulting from any actions, threatened or implied, by the child or their guardians.
f. It is a condition of your enrolment that your child attends a minimum of 2 days per week at the centre and that the enrolment includes either a Monday or a Friday. We are unable to offer an enrolment that does not include a Monday or Friday.

I have read and understood each of the Terms and Conditions of Enrolment specified above. Furthermore I agree to abide by each of these Terms and Conditions of Enrolment without reservation or condition.
I warrant that the information I have provided to Universal Childcare in the Child Personal Profile and Child Enrolment Form and all other information I have provided with respect to my child is to the best of my knowledge and ability true and correct in every respect.
I undertake to inform Universal Childcare immediately should there be any change to this information.
I understand these Terms and Conditions are subject to change without notice, from time to time at Universal Childcare's sole discretion.
I acknowledge that I have read and completed all sections on all pages including this one.
